Steps to register

1. Click on preferred day/time on the calendar to verify the opening. Determine which one works best for you and include it in the 'contact us' form below. (If none of the openings currently work include that you’d like to be placed on a waiting list and include your available days and times).

2. Submit the form using the subject line REGISTER FOR CHASTAIN by clicking the SUBMIT button

3. Download and complete the introductory packet HERE and fax or mail it back to us. Fax # 888-551-6210
​
4. Scan and submit a medical prescription for all PT/OT/SLP sessions as well as copies of your insurance card (front and back) if we are to bill insurance. If you don’t have a prescription call your referring physician and have them fax one to 888-551-6210. If you are coming through a third party source have your case manager contact us

5. You will be contacted to schedule your initial visit with us.
